All job seekers are eligible to use the Centers self-directed
services for their job search needs. Additional services and
training are available to those who have lost their jobs due
to the economy, company downsizing, or layoff. Using a structured,
yet individualized process, the Employment Connection Center in Cleveland, Ohio, provides much-needed support
services developed specifically for people in transition.
NECESSARY
DOCUMENTATION FOR CORE SERVICES ENROLLMENT:

Driver's
License or State ID AND
- Social
Security Card AND
- Proof
of Citizenship and Date/Place of Birth (Birth Certificate,
Unexpired Passport, Resident Alien Card, ISS1 or I151 Form,
or Naturalization papers) AND
- Selective
Service Enrollment Card or Verification (for males born
after December 31, 1959) AND
NECESSARY
DOCUMENTATION FOR ADDITIONAL SERVICES ENROLLMENT:
THE
INFORMATION LISTED BELOW IS DIVIDED BASED ON ELIGIBILITY CATEGORIES.
PLEASE BRING THE DOCUMENTS FOR THE CATEGORY YOU ARE APPLYING
FOR.
Required
by ALL applicants
- Proof
of address (postmarked mail or utility bill [dated in past
30 days] with name and address) AND
- Veteran
status - DD-214 form or Discharge Certificate
For
Adults who are not laid off or displaced homemakers (age 18
and older)
- Proof
of income for last 6 months from self AND
- Spouse
AND
- Blood
relative living in household 18 years of age and older AND
- Parole
release papers (if applicable)
For Laid Off Workers
- Lay off letter printed on company letterhead or any
other document proving lay off such as newspaper article,
company bulletin, severance agreement AND
- Last three pay stubs from your company AND
- Unemployment benefits documentation:
- White Book AND
- Notification of Benefit Rights letter (shows benefit
amount) AND
- Determination of Benefits letter (states whether
claim is allowed or disallowed)
Note: Your most recent check stub from your Unemployment Insurance
payment can be used to substitute for one or both letters.
For Displaced Homemakers
Choose one of the following based on your circumstances:
- Divorce Papers OR separation papers OR
- Spouse's death certificate OR
- Spouse's letter of lay off AND
- If Spouse was laid off, spouse's unemployment benefits
documentation:
- White Book AND
- Notification of Benefit Rights letter (shows benefit
amount) AND
- Determination of Benefits letter (states whether
claim is allowed or disallowed)
Note: A check stub from unemployment check is necessary if
you are unable to find the two letters listed above.
- Last three paycheck stubs from part-time employment
(if applicable) AND
- Letter from spouse verifying you do not work or worked
part time and were supported by spouse's income prior to lay
off.
